**Exciting Opportunity for a Case Manager!**
**Job Description:**
- **Position:** Case Manager
- **Profession:** Registered Nurse
- **Specialty:** Case Manager
- **Shift:** Day 5x8-Hour (08:00 - 16:30)
- **Travel Experience Required:** Yes (can screen if meets experience requirements)
- **Years of Experience Required:** 2 years of acute hospital case management experience
**Key Responsibilities:**
- Care Coordination
- Discharge Planning
- Work in a fast-paced environment with patient ratios of 1:20
- Participate in family meetings and treatment plan development
- Arrange Durable Medical Equipment (DME) and transport independently
**Preferred Experience:**
- Teaching Hospital
- Community Hospital
- Long Term Acute Care (LTAC)
- Trauma Level I and II
**Essential Skills:**
- Familiarity with charting systems, particularly Epic
- Knowledge of regulatory standards and coding (ICD 10, CPT, HIPAA)
- Experience with HMOs and IPAs
**Working Environment:**
- Collaborate with interdisciplinary teams including Physical Therapy, Respiratory Services, and Social Services
- Patient Age Groups: Adults, Geriatrics
