**Job Description:**
- Start date: ASAP
- Required Experience: 2 years of acute hospital case management
- First-timers accepted: Yes (with Clinical’s approval)
- Weekend Requirement: 2 weekends every 6 weeks
- Call Requirement: Not required
- Float Requirement: Yes, between like units based on need
- Local Candidates: Welcome!
- RTO Restrictions: Please include all requests at the time of submittal
- Charting System: Epic
- Parking: Free parking available!
**Key Qualifications:**
- **Travel Experience:** Required, can screen if meets experience requirements
- **Certifications:**
- BLS (Preferred)
- **Skills:**
- Care coordination
- Discharge planning
- Prior authorizations
- Needs assessment/Order DME
- Disease management
- Utilize InterQual Criteria
**Work Settings:**
- Acute Hospital
- Ambulatory (Preferred)
- Behavioral Health (Preferred)
- Home Health (Preferred)
- Insurance Company (Preferred)
- Long Term Acute Care/Rehab/Skilled Nursing (Preferred)
**Patient Care Experience:**
- Patient Ratios: 1:20
- Patient Age Groups: Adults, Geriatrics
- Interdisciplinary Support:
- IV Teams
- Physical Therapy
- Respiratory Services
- Social Services
**Special Procedures/Unit Details:**
- Organize and participate in family meetings
- Development of treatment plans
- Competency in arranging DME, SNF, and ambulance transport
