**Exciting Case Manager Opportunity - Join Our Team!**
**Job Description:**
- Position: Case Manager
- Start Date: ASAP
- Specialty: Registered Nurse - Case Management
- Shift: Day 5x8-Hour (08:00 - 16:30)
- Scheduling Type: Other
- Floating: Yes, based on need
**Key Responsibilities:**
- Coordinating care for patients in a fast-paced acute hospital environment
- Performing discharge planning and ensuring smooth transitions for patients
- Participate in family meetings and collaborate on treatment plans
- Managing DME (Durable Medical Equipment) arrangements and transport logistics
- Engage in disease management and needs assessment
**Experience Requirements:**
- Minimum 2 years of acute hospital case management experience
- Proven record of care coordination and discharge planning
- Familiarity with charting systems, specifically Epic
**Preferred Qualifications:**
- Teaching hospital experience
- Community hospital experience
- Experience in Long Term Acute Care (LTAC)
- Exposure to Trauma Level I and II facilities
**Certifications:**
- BLS (Basic Life Support) certification preferred
**Skills and Knowledge:**
- Competency in Medicare and Medicaid guidelines
- Knowledge of HIPAA regulations
- Experience with ICD-10 coding and DRG (Diagnosis Related Groups)
- Ability to determine medical necessity using evidence-based guidelines
**Patient Interaction:**
- Patient Ratios: 1:20
- Weekend Coverage: Every other weekend (2 weekends every 6 weeks)
- RTO Restrictions: Please include all requests at the time of submittal
- Orientation: 40 hours provided
**Interdisciplinary Support:**
- Work alongside IV Teams, Physical Therapy, Respiratory Services, Social Services, and more
