Responsibilities
- Provide comprehensive nursing care to hospice patients in accordance with individualized care plans
- Perform patient assessments and monitor physical, emotional, and psychosocial needs
- Administer medications, treatments, and pain management interventions as prescribed
- Educate patients and families regarding disease progression, symptom management, and end-of-life care
- Coordinate care with physicians, social workers, chaplains, therapists, and interdisciplinary team members
- Develop and update individualized hospice care plans based on patient needs
- Maintain accurate and timely patient documentation
- Provide emotional support and advocacy for patients and families
- Ensure compliance with hospice regulations, patient safety standards, and agency policies
- Promote comfort, dignity, and quality of life for patients throughout their hospice journey
Requirements
- Active RN License in California
- Minimum 2 years of recent Hospice, Home Health, Palliative Care, or related nursing experience
- Current BLS Certification required
- Hospice and Palliative Credentialing Center (HPCC) certification preferred
- Strong assessment, communication, and patient advocacy skills
- Ability to work independently and manage patient caseloads effectively
- Compassionate approach to end-of-life care and family support
- Excellent organizational and documentation abilities
