With no two patients or days alike, you'll collaborate to provide out-of-the-box, patient-focused strategies. Here are a few of the responsibilities you can expect: Evaluate, diagnose, and manage acute and chronic kidney conditions, including chronic kidney disease (CKD), hypertension, electrolyte disorders, glomerular diseases, and end-stage renal disease (ESRD). Manage patients receiving dialysis treatment and provide comprehensive care plans for those undergoing kidney transplant evaluation and post-transplant management. Collaborate with primary care providers, cardiologists, endocrinologists, and other specialists to ensure comprehensive, coordinated care.
