Job Summary
An acute care facility in Denville is seeking an adaptable, clinically autonomous Labor & Delivery RN for a 13-week nocturnal assignment. This unit operates under strict AWHONN standards of practice, requiring a nurse who can independently manage high-risk and low-risk laboring patients, circulate obstetric surgical cases, and stabilize newborns at delivery. The ideal candidate will possess advanced competency in electronic fetal monitoring interpretation and the ability to pivot rapidly during sudden obstetric crises.
Key Responsibilities
- Comprehensive Obstetric Care: Manage the full peripartum continuum, including high-risk maternal triage, active labor monitoring, epidural management, and immediate postpartum recovery.
- Electronic Fetal Monitoring ($EFM$): Continuously evaluate fetal heart rate patterns, identifying early signs of distress, evaluating uterine contraction frequency/intensity, and implementing corrective nursing actions.
- High-Alert Infusions: Initiate, titrate, and closely monitor specialized maternal medications, including Oxytocin titration protocols and Magnesium Sulfate infusions for preeclampsia management.
- OR Circulation: Circulate for scheduled and emergent Cesarean sections, bilateral tubal ligations, and cervical cerclages within the dedicated L&D surgical suites.
- Neonatal Resuscitation: Provide immediate stabilization and recovery care for newborns at the time of delivery, executing advanced NRP resuscitation algorithms if respiratory depression is detected.
- Documentation & Safety Alignment: Maintain meticulous, legally compliant nursing records of maternal-fetal assessments and interventions in strict compliance with evidence-based standards.
Requirements & Qualifications
- Licensure: Active New Jersey (NJ) State RN License in hand at submission.
- Clinical Knowledge: Strong working knowledge of AWHONN, ACOG, and national patient safety goals for maternal-child health lines.
- Acuity Comfort: Proven comfort working within a Level II obstetric setting, managing moderately complex maternal complications (e.g., gestational diabetes, preeclampsia, preterm labor).

Practicing under AWHONN standards requires rapid, objective categorization of electronic fetal monitoring tracings to guide clinical interventions:
- Category I (Normal): Predictor of normal fetal acid-base status. Requires a baseline rate of $110\text{–}160\text{ bpm}$, moderate baseline variability ($6\text{–}25\text{ bpm}$), and the complete absence of late or variable decelerations.
- Category II (Indeterminate): Not predictive of abnormal fetal acid-base status, but requires close, continuous evaluation. This includes minimal variability, marked variability, or recurrent variable decelerations accompanied by minimal-to-moderate baseline variability.
- Category III (Abnormal): Predictive of abnormal fetal acid-base status. This requires a sinusoidal pattern OR absent baseline variability accompanied by recurrent late decelerations, recurrent variable decelerations, or bradycardia. Immediate intrauterine resuscitation and preparation for an emergent delivery are mandatory.
