Minimum Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active California Physical Therapist license required to start the assignment.
- Current BLS certification from AHA or ARC may be required.
- Home health experience preferred.
- HCHB (HomeCare HomeBase) experience required.
- Experience with productivity expectations in home health, including ability to meet daily point targets (example productivity model: combination of follow-up and Start of Care visits).
Preferred Qualifications
- Experience managing caseloads in community or home health settings.
- Familiarity with Start of Care documentation and home health visit coordination.
Assignment Details
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM.
- Shift pattern: 8-hour shifts, Monday through Friday.
- On-call or weekend requirements: None.
- Territory: Santa Clarita/Valencia coverage expected.
- Productivity expectation: 6 points per day (may include combinations such as 5 follow-up visits, 2 Start of Care visits, or 1 Start of Care and 2 follow-ups).
