**Key Responsibilities:**
- Develop personalized care plans in collaboration with patients, their families, and physicians upon admission.
- Periodically review and update care plans to ensure optimal patient outcomes.
- Coordinate the care plan through effective communication with interdisciplinary team members and case conferences.
- Conduct ongoing assessments of clinical, functional, and emotional patient status to identify any issues or progress.
- Communicate significant changes to physicians and colleagues promptly.
- Document evaluations, patient responses, and progress towards healthcare goals.
- Educate patients and caregivers on care techniques and end-of-life symptom management as appropriate.
**Minimum Qualifications:**
- Must be a licensed Registered Nurse in Vermont and New Hampshire or possess an unencumbered compact license.
- Graduate of an accredited school of nursing.
- Medical, Surgical, Oncology nursing, or hospice care experience preferred.
