Job Description
We are seeking an experienced Registered Nurse (RN) to join an Acute Care Palliative Care team for a 13-week contract assignment. The RN will provide compassionate, patient-centered care to individuals with serious illnesses while supporting patients and families through goals-of-care discussions, symptom management, advance care planning, and end-of-life decision-making.
The ideal candidate will possess strong clinical assessment skills, exceptional communication abilities, and experience working in acute care, palliative care, hospice, ICU, or oncology settings.
Key Responsibilities
- Review patient charts and develop daily patient caseloads.
- Conduct comprehensive patient assessments and identify care needs.
- Facilitate goals-of-care discussions with patients and families.
- Provide education regarding disease processes, treatment options, advance directives, and POLST documentation.
- Coordinate and participate in family meetings and care conferences.
- Offer emotional support and guidance during end-of-life care discussions.
- Collaborate with physicians, nurse practitioners, and interdisciplinary team members.
- Attend daily rounds and contribute to care planning.
- Document patient interactions and care plans accurately in the EMR.
- Advocate for patient-centered care and support quality-of-life goals.

Requirements
- Active California RN License.
- Bachelor of Science in Nursing (BSN) required.
- Minimum 2 years of recent nursing experience required.
- Recent experience in Palliative Care and/or Hospice strongly preferred.
- Acute Care experience required.
- ICU and/or Oncology experience preferred.
- Current BLS Certification required.
- ELNEC (End-of-Life Nursing Education Consortium) certification preferred.
- Strong clinical judgment, communication, organizational, and critical-thinking skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
