**Join Our Team as a Clinical Care Registered Nurse!**
**Job Description:**
- Step into a pivotal role as a Clinical Care Registered Nurse (CCRN) and provide direct nursing care to participants in a dynamic environment.
- Work independently and apply your advanced nursing knowledge and skills to manage complex clinical situations.
- Collaborate with both licensed and non-licensed personnel to ensure the best care possible.
**Key Responsibilities:**
- Triaging emergent situations while utilizing advanced nursing knowledge.
- Monitoring both acute and chronic medical conditions in sync with Primary Care Providers (PCPs).
- Utilizing care plans independently for clinic and home-based direct care.
- Administering treatments, procedures, and medications as prescribed by the PCPs.
- Conducting regular home visits to deliver care and treatment, which may include:
- Wound care
- Lab draws
- Nurse assessments
- Other nursing tasks
**Qualifications:**
- Bachelor's degree preferred in Science of Nursing; Associates degree required.
- RN Nursing Diploma or Degree from an accredited School of Nursing or University.
- Minimum of one year of experience working with the frail, elderly, or long-term care population.
- Minimum of one year of home care experience preferred.
- Minimum of one year of wound care experience preferred.
**Specific Requirements:**
- Current CPR certification by AHA or willingness to obtain it through Senior Care Partners PACE.
- Demonstrates core values of compassion, integrity, teamwork, and excellence.
**Work Environment:**
- Has a visible presence while working and is subject to frequent interruptions.
- Engages in effective communication with staff, participants, caregivers, and other organizations as necessary.
- Flexibility to work beyond normal hours, including weekends and holidays as needed.
**Minimum Working Requirements:**
- Must possess reliable transportation, a current valid Michigan driver’s license, and car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated or willing to be fully vaccinated against communicable diseases, including:
- Influenza
- MMR
- Varicella
- Tdap/Td
- Covid-19
- Hepatitis B
