**Job Description:**
- **Position:** Case Manager RN
- **Experience Required:** 2 years in an acute care setting
- **Travel Experience:** Not required
- **Shift:** Day shift, 5x8-Hour (07:30 - 16:00)
- **Weekend Coverage:** Every other weekend
- **Admission Criteria:**
- Care coordination
- Continued stay reviews
- Determine medical necessity per evidence-based guidelines
- Needs assessment/order DME
- Plan of care
- Pre-cert review and prior authorizations
- Utilize InterQual criteria and Milliman guidelines
- Workers' compensation case management
- **Certifications:**
- ACM-RN (preferred)
- AK RN License (required)
- BLS (preferred)
- **Required Skills:**
- Epic charting system experience
- Current AK RN license in hand for auto offer
- Must have acute care experience
- **Patient Age Groups:**
- Neonates
- Adolescents
- Infants
- Adults
- Pediatrics
- Geriatrics
- **Daily Census:** 324 patients with a total of 401 beds
- **Interdisciplinary Support:**
- IV Teams
- Physical Therapy
- Respiratory Services
- Social Services
- Pharmacy

- **Key Responsibilities:**
- Discharge planning
- Cover floating responsibilities within case management services as needed
- Adhere to CMS guidelines and HIPAA regulations
- Collaborate with multiple departments for effective patient care
- **Orientation Hours:** 24
- **Work Settings:**

- Case management/utilization review
- **Schedule Flexibility:** No block schedules available
