**Join Our Compassionate Team as a Home Care RN!**
**Job Description:**
- Provide hospice coverage when on-call and make a positive impact on patients' lives.
- Complete OASIS documentation accurately and timely, in line with agency policies and regulations.
- Utilize the nursing process to assess patient and caregiver needs, delivering quality care based on evidence-based practices.
- Develop and share realistic care plans with patients and caregivers, ensuring their involvement in the process.
- Act as a Case Manager, actively managing patient caseloads and visit times, ensuring agency productivity standards are met.
- Implement medical plans of care and nursing interventions, prioritizing safe and effective patient care.
- Stay current on high-tech skills including IV use, wound VAC, and complex wound care, while maintaining infection control standards.
- Collaborate with LPNs to follow the care plan, delegate tasks, and ensure all patient needs are addressed.
- Evaluate the effectiveness of care plans, adjusting as needed to meet patient and caregiver requirements.
- Maintain accurate and up-to-date documentation to facilitate ongoing quality care among the healthcare team.
- Follow agency protocols with McKesson processes, ensuring efficient visit documentation and completion.
- Provide patient and caregiver education to support informed decision-making on treatment options and care instructions.
- Identify interdisciplinary needs for patients and effectively communicate those needs to the relevant teams.
- Complete necessary agency, state, and federal forms accurately and efficiently during visits.
- Conduct visits in a timely manner, ensuring proper scheduling and communication with the Scheduler.
- Assess patients' needs for Home Care Aide services, coordinating care plans with patients and caregivers, providing ongoing supervision.
- Transmit patient incident reports to the Quality Improvement Specialist promptly as necessary.
- Coordinate safe patient discharges or transfers, ensuring all paperwork and patient information is processed correctly.
- Collaborate with physicians, providers, and team members to support recommended care plans, ensuring safe patient and family care.
- Participate in monthly peer review audits, offering and accepting feedback in a professional manner.
- Schedule and facilitate case conferences with managers and team members to enhance team communication.
- Advocate for patient care issues to ensure quality care sensitive to individual patient and family needs.
**Shift Information:**
- Tuesday - Saturday, 8 am - 4 pm
- On-call rotation required
