**Exciting Opportunity: Registered Nurse Case Manager - Primary Care/Clinic Nurse**
**Job Description:**
- **Make a Difference:** Join our vibrant healthcare team and actively manage patient care for SEARHC patients while serving as a clinic nurse!
- **Collaboration:** Work alongside medical providers and interdisciplinary team members to set individualized medical goals that enhance patient health outcomes.
- **Patient Advocacy:** Help high-risk and complex patients through coordinated multi-disciplinary care conferences, advocating for their needs and fostering self-care.
- **Proactive Management:** Oversee acute and chronic care needs by reviewing EHR reminders, performing outreach, and providing patient education.
- **Team Engagement:** Participate in case management reviews with a multi-disciplinary team, ensuring comprehensive care for all high-need patients.
- **Referral Coordination:** Manage patient referrals effectively, monitoring the process and resolving any delays to ensure timely care.
- **Nursing Excellence:** Perform essential nursing duties such as assessment, documentation, education, and medication administration while maintaining safety and infection control standards.
- **Communication:** Build strong relationships across departments and contribute to team improvement efforts, being a leader among nursing staff.
- **Continuous Growth:** Be a part of developing the case management role in primary care and participate in emergency response as needed.
- **Diverse Tasks:** Perform various nursing functions and take on special projects that enhance our health services.
**Qualifications:**
- Graduate of an accredited nursing program; Bachelor's degree preferred.
- Active, unrestricted Registered Nursing license in Alaska required.
- Basic Life Support certification; National Case Management certification preferred.
- Minimum of 1 year of case management experience within the last 2 years.
- Strong knowledge of nursing processes, customer service principles, and safety/infection control standards.
- Proficient in assessment and data collection with excellent oral and written communication skills.
- Ability to multi-task and work independently in a fast-paced environment.
- Strong problem-solving skills and ability to maintain positive working relationships.
- Capable of performing physical tasks related to nursing activities.
