Minimum Requirements

Associate's, Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
Current Physical Therapy licensure in South Carolina required to start the assignment.
Current CPR certification required.
Current driver's license, vehicle insurance, and access to a dependable vehicle or reliable public transportation for patient visits.
Experience providing home health physical therapy preferred, including patient evaluations, developing PT plans of care, progress documentation, and coordinating with interdisciplinary teams.
Ability to perform patient evaluations, develop and implement treatment plans, and document outcomes per agency policy and state guidelines.

Assignment Details

Facility Type: Home Health.
Shift Name: Days.
Shift Details: Days 0800-1600.
Weekly Hours: 40 hours per week (5x8 schedule).
Coverage Areas: Chesterfield County, Darlington County, and Marlboro County, SC.
Mileage reimbursement provided between first and last patient visit.
