**Job Description:**
- Are you ready to make a meaningful impact on the lives of individuals and families facing end-of-life challenges?
- We are seeking a dedicated Registered Nurse to provide personalized, one-on-one care in the comfort of patients' homes.
**Your Responsibilities:**
- Provide compassionate end-of-life care, ensuring the physical and emotional well-being of patients and their families.
- Deliver expert pain and symptom management, offering comfort and support during challenging times.
- Collaborate closely with our interdisciplinary team, local physicians, and community providers to optimize the quality of life for patients.
**What We’re Looking For:**
- Minimum of 1 year of experience in home health care.
- OASIS experience is a must.
- Home health experience required.
- Ability to perform case management effectively.
- Certifications: BLS/BCLS.
**Shift Information:**
- Days: Monday to Friday with some weekend coverage.
