Requirements:

Active RN license in the state of practice.
Minimum of 1–2 years of nursing experience, preferably in hospice, palliative care, or home health.
Strong assessment, clinical, and communication skills.
Knowledge of hospice regulations, standards, and care planning.

Responsibilities:

Assess, plan, implement, and evaluate individualized hospice care for patients.
Coordinate care with physicians, nurses, social workers, and other healthcare professionals.
Provide education and support to patients and their families regarding disease progression and end-of-life care.
Ensure documentation complies with hospice standards and regulatory requirements.
Monitor patient conditions and adjust care plans as necessary.
Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.
