Requirements
Roles & Responsibilities
- Perform comprehensive patient assessments in home health and hospice settings
- Develop, implement, and evaluate individualized plans of care
- Provide skilled nursing services and patient education
- Monitor patient progress and communicate changes to physicians and care teams
- Coordinate care with interdisciplinary healthcare professionals
- Manage medication administration and treatment plans
- Educate patients and caregivers regarding disease management and care goals
- Maintain accurate and timely electronic medical record documentation
- Support end-of-life care and hospice patients with compassion and professionalism
- Promote patient safety, independence, and quality of life in the home setting
Qualifications
- Active New Hampshire Registered Nurse (RN) License required
- Current American Heart Association (AHA) BLS Certification required
- Minimum 18 months of current Home Health experience required
- Previous Hospice experience preferred
- 2–5 years of Medical-Surgical nursing experience preferred
- Strong assessment, clinical judgment, and care coordination skills
- Ability to work independently with minimal supervision
- Computer proficiency and EMR documentation experience required
- Valid Driver’s License required
- Current Auto Insurance required
- Reliable transportation required
