Minimum Requirements
Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by APTA. Current, unencumbered Physical Therapy license for the State of Indiana required to start the assignment. Recent home health experience required. Must demonstrate proficiency in home health clinical processes and OASIS visits (Start of Care, Resumption of Care, Evaluation, Discharge). Proficiency with HomeCareHomeBase or similar home health EMR preferred. Understanding of Medicare guidelines for home health required. BLS/CPR certification required. Valid driver’s license, dependable vehicle, and proof of vehicle insurance required. Mileage reimbursed between visits. Ability to document within 24 hours of visit and to supervise PTA care per company policy and state regulation when applicable. Ability to participate in weekend rotation and potential availability on observed holidays. Every third Saturday rotation required.

Facility Type: Home Health. Shift Name: Days. Shift Details: Monday–Friday, 8:00 AM–4:00 PM (days). Weekly Hours Guarantee: 36 hours per week. Productivity Expectation: 30 points per week. Coverage Area Primary: Wabash, Huntington, and Grant Counties. Must be able to service Blackford, Delaware, and Madison Counties as needed. On-call/Weekend Requirement: Every 3rd Saturday rotation required. Dress Code: Navy blue scrubs. Role Type: Independent Contractor supporting Care Services, Clinical segment.
