Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist role in Shelbyville, KY, providing home health services to patients recovering from injuries and managing chronic conditions. This position requires completing SOCs/ROCs, routine visits, supervisory visits, and re-assessments while documenting in OASIS and HCHB.

Minimum Qualifications

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Kentucky Physical Therapist license required to start the assignment.
Home health experience required.
Experience with OASIS and HCHB required.
BLS certification from AHA or ARC may be required.
OASIS certification preferred.
Must be able to complete SOCs/ROCs, routine visits, supervisory visits, and re-assessments.

Preferred Qualifications

OASIS certification.

Assignment Details

Shift hours: Monday–Friday, 8:00 AM–4:30 PM.
Weekly guaranteed hours: 32.
On-call or weekend requirement: No.
Will be reimbursed for mileage between visits from first patient to last patient.
Coverage areas include Shelby County, eastern Jefferson County, and Spencer County.

The city is near Shelbyville Country Club and several local parks, and is approximately 30 minutes east of downtown Louisville. The area provides access to the Kentucky Bourbon Trail within an hour’s drive and to state parks such as Bernheim Forest about 25 miles away, supporting an active outdoor lifestyle and short commutes to urban amenities.
