Minimum Qualifications
Associate degree in Physical Therapy from an accredited educational program.
Active Illinois PTA license required to start the assignment.
BLS certification from AHA or ARC may be required.
Experience working in a skilled nursing facility or long-term care setting preferred.
Prior experience providing care to older adults and working with common SNF equipment (gait belts, Hoyer lifts, walkers) preferred.
Ability to work collaboratively with interdisciplinary team and follow PT-directed plans of care.
Assignment Details
Facility Type: Skilled Nursing Facility.
Shift Name: Days.
Shift Details: Days 08:30–16:30.
Schedule: Five 8-hour shifts per week.
Floating: Candidate may be required to float to other facilities as needed.
