**Key Details:**
- **Position:** Registered Nurse, Case Manager
- **Shift:** Day shifts (5x8 hours, 08:00 - 16:30)
- **Experience Required:** 2 years of acute hospital case management experience, including care coordination and discharge planning.
- **Travel Experience Required:** Yes (First-time travelers welcome with Clinical approval)
- **Weekend Coverage:** Every other weekend
**Responsibilities:**
- Perform effective care coordination and discharge planning for patients.
- Adhere to admission criteria, appeals, and denials management.
- Support regulatory compliance (CMS, HIPAA, The Joint Commission).
- Engage with interdisciplinary support, including social services.
**Skills Required:**
- Proficient in Epic charting system.
- Experience with HMOs, IPAs, advance directives, POLST, and protective services processes.
- Comfortable floating to meet department needs.
- Ability to manage a case load of up to 25 patients.
**Patient Demographics:**
- Work with adolescents, adults, pediatrics, and geriatrics.
- Daily patient ratios of 1:20, ensuring quality care for each individual.
