**Weekend Field Nurse Opportunity**
**Job Description**
- Seeking a passionate and dedicated Weekend Field Nurse to join our dynamic team!
- Primary focus: Conduct participant assessments to determine appropriate levels of care.
- Respond promptly to communications from primary care providers and support staff.
- Perform regular field visits to deliver essential nursing care, including:
- Wound care
- Health assessments
- Collaborate with various stakeholders for comprehensive participant support.
- Document interactions thoroughly and initiate emergency treatments as needed.
- Engage in quality improvement initiatives in a participant-centered care framework.
- Adapt to changing workloads and maintain effective communication with leadership.
**Key Responsibilities**
- Conduct in-person assessments to ensure medical needs are met safely.
- Prioritize and respond quickly to calls from primary care providers (PCP) and support staff.
- Make regular home visits to provide care, treatment, and education, including:
- Wound care
- IV hydration
- Lab draws
- Educate participants, families, and facility staff about care.
- Coordinate care among providers, suppliers, and support teams.
- Initiate and document new orders per necessity, updating relevant documentation.
- Complete reports for falls, medication errors, incidents, and quality measures.
- Collaborate with staff, participants, and families using a participant-centered care model.
- Participate in orientation, in-service programs, staff meetings, and quality improvement initiatives.
- Adjust to changing workload and schedule priorities effectively.
- Keep leadership informed of any issues or concerns.
**Qualifications**
- Bachelor’s degree in Science of Nursing preferred; Associate's degree required.
- RN Nursing Diploma or Degree from an accredited institution.
- Minimum 1 year of experience with the frail elderly or long-term care population.
- Home care experience preferred.
- Wound care experience preferred.
**Specific Requirements**
- Current CPR certification or willingness to obtain.
- Demonstrate core values of compassion, integrity, teamwork, and excellence.
**Work Environment**
- Visible presence while working, subject to frequent interruptions.
- Communicate effectively with staff, participants, caregivers, and other organizations.
- Flexibility to work beyond normal hours, including weekends and holidays.
**Minimum Working Requirements**
- Reliable transportation and valid driver’s license.
- Must be 18 years of age or older.
- Must be fully vaccinated or willing to be vaccinated against communicable diseases.
