**Exciting Opportunity for a Compassionate Hospice RN!**
Join our dedicated team focused on providing compassionate, high-quality end-of-life care to patients and families! We are looking for a Hospice Registered Nurse (RN) to make a meaningful impact in the lives of others.
**Key Responsibilities:**
- Perform comprehensive assessments of patients’ needs
- Coordinate individualized care plans tailored to each patient
- Provide pain and symptom management in line with the hospice care philosophy
- Educate and support patients and families on the disease process, self-care techniques, and end-of-life issues
- Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and volunteers
- Participate in on-call rotation to respond to patient needs
- Maintain accurate and timely documentation per state, federal, and organizational policies
**Qualifications:**
- Current Registered Nurse (RN) license in the state of Utah or compact state RN license
- Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care
- Strong clinical assessment skills
- Excellent communication, interpersonal, and organizational abilities
- Compassionate demeanor and capability to support patients and families during challenging times
- Valid driver’s license and reliable transportation required for home visits
- Current BLS Certification
- Experience with Hospicesoft/careficient EMR preferred
