**Join Our Team as a Registered Nurse - Case Manager!**
**Job Description:**
- **Position:** Registered Nurse - Case Manager
- **Travel Experience Required:** Yes, seasoned travelers preferred!
- **Shift:** Day 5x8-Hour (08:00 - 16:30)
- **Patient Ratios:**
- Days: 1:25
- Weekends: 1:45
- **Years of Experience Required:** 2 years in Acute Care Case Management/Discharge Planning
- **Charting System:** Epic Required
- **Location:** Acute Hospital (No Floating Expectation)
- **Orientation Hours:** 40
- **Patient Load:** Up to 25 patients
- **Age Groups:** Neonates, Adolescents, Adults, Geriatrics
**Key Responsibilities:**
- Conduct thorough patient assessments.
- Coordinate care effectively with interdisciplinary teams.
- Lead discharge planning and ensure smooth transitions of care.
- Participate actively in family meetings.
- Develop and implement treatment plans.
- Navigate HMOs, IPAs and manage complex patient needs.
- Secure necessary DME and arrange SNF and ambulance transport independently.
**Essential Skills:**
- Expertise in Care Coordination and Utilization Review.
- Knowledge of regulatory frameworks including CMS and the Joint Commission.
- Familiarity with patient support processes such as adoption/foster care and mental health resources.
**Additional Information:**
- Weekend rotation required to meet departmental needs (every other weekend).
- Must hold a valid CA state license or be pending but licensed upon start date.
- 1 request for pre-approved time off allowed.
- Strong candidates will not be overlooked even if they are missing some required skills or experience.
