**Join Our Dynamic Team as a Case Manager!**
**Job Description:**
- **Position:** Case Manager
- **Start Date:** ASAP
- **Shift:** Day 5x8-Hour (08:00 - 16:30)
- **Patient Ratio:** 1:20
- **Weekend Requirement:** 2 weekends every 6 weeks
- **Call Requirement:** No
- **Floating:** Yes, based on need
- **Local Candidates Allowed:** Yes
- **Parking:** Free Parking!
**Key Qualifications:**
- **Experience:** Minimum of 2 years in acute Hospital Case Management
- **Background in:** Care Coordination, Discharge Planning in fast-paced environments
- **Charting System:** Epic (required)
- **Certifications:** BLS (preferred)
**Preferred Experience in:**
- Teaching Hospitals
- Community Hospitals
- Long Term Acute Care (LTAC)
- Trauma Level I & II
**Skills Required:**
- Care Coordination
- Discharge Planning
- Determine Medical Necessity per Evidence-Based Guidelines
- Needs Assessment / Order DME
- Prior Authorizations
- Experience with HMOs, IPAs, etc.
- Competent in arranging DME, SNF, and ambulance transport
**Interdisciplinary Support:**
- IV Teams
- Physical Therapy
- Respiratory Services
- Interpretation Services
- Social Services
- Rapid Response Teams
**Additional Requirements:**
- Ability to organize and participate in family meetings
- Participate in treatment plan development
- Must have current work history within 2 years (HARDSTOP)
