Requirements:

Current RN license in the state of practice.
Minimum of 1–2 years of nursing experience; home health experience preferred.
Strong clinical assessment, critical thinking, and communication skills.
Ability to work independently and manage multiple patient visits efficiently.
Valid driver’s license and reliable transportation.
Basic Life Support (BLS) certification required; other certifications may be preferred.

Responsibilities:

Assess patient health status, develop individualized care plans, and evaluate outcomes.
Provide nursing interventions including medication administration, wound care, and chronic disease management.
Educate patients and families on disease management, medications, lifestyle modifications, and self-care techniques.
Monitor and document patient progress, communicate changes to physicians, and coordinate care with interdisciplinary teams.
Ensure compliance with infection control, safety protocols, and regulatory standards.
Support patients and families emotionally and provide guidance on home care resources.
Participate in quality improvement initiatives and professional development activities.
