Requirements:

Must hold active LPN/LVN license in practicing state.
LTC/SNF/Assisted Living experience preferred.
Strong clinical, communication, and care skills.
Able to follow care plans and collaborate with staff.

Responsibilities:

Clinical Care:
- Assist with ADLs and monitor resident health status.
- Administer medications and treatments.
- Document resident care and report all changes.

Collaboration & Safety:
- Follow physician orders and established care plans.
- Promote a safe, clean, and supportive environment.
- Provide emotional support and companionship.
