Job Description:
Pagosa Springs Medical Center is seeking an experienced Travel Oncology Nurse Navigator to support our Oncology and Infusion Services team. This role combines oncology patient navigation with direct outpatient oncology nursing support. The Nurse Navigator serves as a central point of coordination for oncology patients throughout diagnosis, treatment, and follow-up. The position also supports infusion and clinic workflows, ensuring patients receive timely care, education, and care coordination across the continuum.
Responsibilities:
- Coordinate care for oncology patients across the treatment continuum
- Provide patient education regarding diagnosis, treatment plans, and symptom management
- Collaborate with oncology providers and interdisciplinary care teams
- Assist with infusion clinic operations as needed
- Perform IV starts and support port/PICC line access when required
- Support chemotherapy/biotherapy treatment workflows
- Coordinate imaging, specialty referrals, and prior authorizations
- Assist with oral chemotherapy medication coordination and patient monitoring
- Provide psychosocial support and connect patients with community resources
- Maintain accurate clinical documentation and care coordination notes
Requirements:
- Active Registered Nurse license
- Minimum 2 years of Oncology Nurse Navigator experience
- Strong oncology nursing background
- ONC certification required
- Experience working with oncology patients in an outpatient or infusion setting
- Strong patient education and care coordination skills
