Job Description:
Pediatric RN Case Manager providing in-home hospice and concurrent care services for patients birth to 21 years. Responsible for care coordination, skilled nursing assessments, symptom management, and family education. 4-5 home visits per day.

Requirements:
- Active CA RN License
- Minimum 2 years pediatric experience (PICU, NICU, pediatric acute care, oncology, and/or hospice)
- Hospice or case management experience preferred
- BLS required
- Reliable transportation
- Valid driver's license
- Current auto insurance with acceptable coverage
- Current vehicle registration
