We are inviting Internal Medicine physicians to join a team expanding care beyond medicine for the most vulnerable patient population. This is a rapidly growing Colorado-based physician-owned practice with current openings in Fort Collins, Greeley, Loveland, Windsor, CO.

Practice Details

Round on patients in skilled nursing facilities and long-term care
Team approach coordinating with specialists and surgeons, Physical, Occupational & Speech Therapies, Case Management, and Psychology
Clinical Triage manages calls from facilities 7 days a week (no overnight call)
Providers utilize smart technology with integrated voice recognition to complete patient encounters quickly and directly into EHR
Guaranteed base salary along with bonus incentives (earn 275K -300K) and robust benefits
