Position Overview
The Case Management unit at Upper Chesapeake Medical Center provides inpatient case management services, coordinating medical home care, placement, and social support for safe discharges. This role requires strong communication and coordination skills across interdisciplinary teams to optimize patient outcomes and compliance with regulatory requirements.
Requirements
Licensure:
Active Maryland RN License or Compact License – Required at submission
Experience:
Minimum 2 years of inpatient RN experience – Preferred
3 years of case management experience preferred but will accept strong 2-year candidates or first-time travelers
Travel Experience:
First-time travelers – Accepted
Local Radius Rule:
Locals within 50 miles – Not Accepted
Teaching Hospital Experience: – Not required
Community Hospital Experience: – Preferred
Certifications
- BLS (AHA) – Preferred
- CPI, ACLS, PALS, NRP – As required for compliance
