Position Overview
Seeking a compassionate and experienced Registered Nurse – Diabetes Educator (CDCES) to join our multidisciplinary diabetes management team. This role focuses on providing holistic, patient-centered care and diabetes education to patients and families living with diabetes mellitus.
The ideal candidate will possess strong clinical judgment, patient advocacy skills, and a passion for education and empowerment in diabetes care.
Education and Experience
- Graduate of an accredited nursing program
- BSN preferred
- Experience in diabetes education or endocrinology preferred
Certification and Licensure
- Active RN license in both Illinois and Wisconsin
- Approved CPR certification (required within 90 days of hire)
- Certified Diabetes Care and Education Specialist (CDCES) certification required
Essential Duties and Responsibilities
- Utilize the nursing process and collaborate with interdisciplinary teams to provide exceptional patient care.
- Administer prescribed medications and treatments per approved nursing techniques, policies, and state practice acts.
- Accurately document all patient care activities in accordance with established standards.
- Develop, initiate, and implement individualized plans of care in collaboration with physicians and multidisciplinary teams.
- Participate in departmental performance improvement activities and shared governance initiatives.
- Provide guidance and clinical expertise to other units/departments as part of the patient’s longitudinal care plan.
- Apply critical thinking to enhance patient safety and experience, escalating concerns appropriately.
- Act as a resource for staff on patient education programs and materials.
- Serve as a patient advocate, addressing clinical, psychosocial, and financial issues as they arise.
- Participate in ongoing professional development and continuing education in diabetes management.
