NOTE: This is for the Bronx Borough. Patient range from 18 years old to 100 years old. THIS ROLE IS FOR TRUE AT HOME, nurses must travel to patients who come within the Bronx borough to provide care.
Flexible with days of work it can be: Sunday to Wed, Thursday to Saturday etc. but must do consistent days.
Alternating weekends are required.
Pay Range:
Local: (Within 50 miles radius rule)
Hourly: $70 - $80
Weekly : $3000
Travel: (Resides 50 miles)
Blended Hourly: $80 -$92
Weekly Stipend: $2400
Weekly Gross: $3100 - $3400
Position Summary:
We are seeking an experienced Registered Nurse (RN) with a minimum of 2 years of acute care experience to join our Adult Care Home Health team. The ideal candidate will provide compassionate, high-quality care to adult patients in a home health setting, ensuring individualized care plans and supporting patients in achieving their optimal health outcomes.
Experience Required:
2 years of previous acute care experience in Med/Surg, Maternal and Child Care, Critical Care, Pediatrics. -Current Adult Care home health experience.
Roles & Responsibilities:
- Provide skilled nursing care and health assessments to adult patients in their homes.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Administer medications, monitor patient progress, and document care accurately.
- Educate patients and families on disease management, medications, and self-care techniques.
- Coordinate with physicians and other healthcare professionals to ensure continuity of care.
Skills:
- Strong clinical assessment and critical thinking skills.
- Excellent communication and patient education abilities.
- Proficiency in medication administration and care documentation.
- Ability to work independently and manage multiple home visits efficiently.
- Compassionate, patient-centered approach with strong interpersonal skills.

- Valid NY State RN license
- ACLS Certified through AHA
- BLS Certified through AHA
