Position Title: Regional Transition Administrator
Salary Range: Regional Grade 15, Exempt. Annual salary range: $77,904 - $97,380 per fiscal year. This range reflects the minimum to midpoint of the grade. Salary may exceed the midpoint and will commensurate with experience.
Job Description Summary: The Regional Transition Administrator is responsible for providing comprehensive case management services and identifying transition options for clients in need of alternative placements or discharge from a formal placement This role includes locating suitable housing, offering advice to multiple counties, and identifying process efficiencies to improve service delivery across Region 5 + (comprised of the Leech Lake Band and Mille Lacs Band of Ojibwe and Aitkin, Cass, Crow Wing, Morrison, Todd, and Wadena counties). Additionally, this role serves as an advocate for the clients they serve as well as the participating Region 5+ Counties and Tribal communities.
Essential Duties and Responsibilities 1. Provide Region 5+ Case Management: - Monitor the individual's progress in treatment.
- Manage the client discharge plans.
- Identify and build relationships with contacts at treatment facilities.
- Provide liaison relationship between Region 5+ Counties and MHSATS facilities.
- Support and work collaboratively with County case managers to locate inpatient treatment availability.
- Work to ensure individuals find placement in the community before they do not meet criteria at an inpatient setting.
- Meet with point of contact for each county at least once quarterly to discuss caseload and to provide case consultation.
- In conjunction with county social services, represent Region 5+ in meetings with DHS regarding matters of placement and individuals served by MHSATS.
- Assist county case managers with strategizing and implementing transition plans for individuals served.
- Assist count case managers in advocating for county perspective in interactions with stakeholder agencies.
2. Pre-Admission Case Management Services: - Process notifications of requests for Transitional Services (referrals).
- Provide consultation to the County case manager(s) to alleviate the need for commitment, hospitalization, or alternative placement.
- Provide consultation and assistance with the commitment process when necessary.
- Assist with admission process to facilities identified by the case manager or the Transition Specialist as potential resources for the client.
3. Provide Post-Admission Case Management Services: - Lead efforts in proactive and timely planning for discharge.
- For individuals served by MHSATS, make contact as needed, not less than every other week with the care coordination teams, county case managers and/or individuals receiving care to discuss status and potential options for discharge.
- Determine who should be on the client's treatment team (involved parties) for case consultation and discharge planning purposes.
- Ensure completion of shared planning documentation, and that all treatment team members receive updated information.
- Attend team meetings in person, by phone and/or via ITV as needed at the inpatient setting where the individual is in order to discuss the course of treatment of the individual's and process potential discharge ideas.
- Identify resources available for discharge in the "home" community and across the state, for potential options for discharge.
- Work to develop individualized services needed, but not yet available.
- Work to build knowledge and relationships with placing facilities to have a wide range of options available for case management consults.
- Complete all documentation, assessments, goal plan, discharge plan and make referrals as needed.
- Work collaboratively with the county case manager and anticipated providers to assure smooth transition and ongoing ability to provide appropriate level of care.
4. Provide Post-Discharge Case Management Services: - Provide coordination duties for up to two weeks following discharge from the hospital.
- Continue collaborative communication with involved parties.
5. Provide advisory services to Region 5+ Counties and tribal communities. - Host, coordinate, and facilitate the quarterly AMH Case Manager's Network.
- Provide advice and support on best practices for transitioning individuals
- Serve as liaison between counties, ensuring consistent and high-quality service delivery
6. Evaluate and identify process efficiencies to enhance service delivery across multiple counties. Represent Region V+ AMHI and Sourcewell in matters pertaining to individuals needing placement. - Attend community meetings requested by AMHI members.
- Attend and present at DHS and MACSSA meetings as requested.
- Attend and present at County Board Meetings as requested.
- Provide community engagement and education as needed and requested by the AMHI.
- Assist Community Health team in planning, organizing, and facilitating regional mental health meetings such as Community Bridgemakers and other regional mental health meetings
7. Other duties as assigned Additional Job Description: Required Qualifications - Be licensed as a Social Worker (Bachelor of Social Work required for licensure) or licensed as a Graduate Social Worker (Master's in Social Work required for licensure) as stated by the Minnesota Board of Social Work
OR
- Hold a 4-year degree in Human Services, Psychology or Sociology, or related fields.
- Five Years' experience n social services, and at least three (3) of those years' experience in a public welfare agency in the specific unit or area of specialty.
- Valid MN Driver's License

- Preferred applicants should reside in Minnesota and be located near or in the counties we serve (Region 5).
- You must be able to commute throughout the region at least 3 days a week to make connections and develop relationships within our communities.
