Description
We are seeking a dedicated and detail-oriented MDS Coordinator to join our team at Colonial Vista Post Acute. The MDS Coordinator will play a vital role in ensuring compliance with state and federal regulations regarding resident assessments in long-term care facilities. This position requires a strong understanding of the Minimum Data Set (MDS) assessment process, as the coordinator will be responsible for the accurate and timely completion of assessments. The ideal candidate will possess excellent communication and organizational skills, allowing them to effectively collaborate with healthcare professionals, nursing staff, and management to optimize resident care plans. By utilizing their expertise, the MDS Coordinator will help enhance the quality of care delivered to our residents, ensuring they receive the highest standards of service. This role also involves educating and training staff on MDS-related policies and procedures, making it essential that the MDS Coordinator stays updated on industry standards and regulations. Responsibilities

Coordinate and oversee the MDS assessment process to ensure accurate and timely completion of resident assessments.
Review resident clinical data and collaborate with the interdisciplinary team to develop and implement appropriate care plans.
Ensure compliance with state and federal regulations related to MDS assessments and documentation.
Provide training and support to nursing staff and other team members on MDS procedures and best practices.
Prepare and submit accurate reports to management regarding MDS compliance and quality measures.
Monitor and analyze MDS data to identify trends and areas for improvement in resident care.
Serve as a liaison between regulatory agencies and the facility concerning MDS-related issues.

Requirements

Bachelor's degree in Nursing, Health Administration, or a related field.
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license is preferred.
Proven experience in long-term care or skilled nursing facilities is essential.
Thorough understanding of MDS regulations, processes, and documentation requirements.
Exceptional attention to detail and strong analytical skills.
Excellent verbal and written communication skills for effective interaction with staff and residents.
Ability to work independently while managing multiple priorities and deadlines.
