Outpatient Home Health Physical Therapist inSenior Living Communities
BAYADA Home Health Careis currently seeking an experiencedPhysical Therapistfor aFull Timeopportunity performingoutpatient visitswithinSenior Living Communities.
This Physical Therapist will treat geriatric clients who are residents in Assisted Living and Independent Living Communities. You will be instrumental in optimizing function and independence for older adults through proactive evidence based interventions.
You will also have theoptionof leading group fitness classes andhaveoversight of fitness offerings within Senior Living Communities, plus you willparticipate in client screens and wellness events.
One year prior clinical experience as a licensed Physical Therapist is required. As anoutpatient Physical Therapist,you will be an integral member of a multi-disciplinary health care team that providesoutpatient services to clients affording them the opportunity to age in place.
Responsibilities for an Outpatient Physical Therapist include:
- Makeoutpatientvisits to clients in designated senior living communities.
- Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise the Physical Therapy - PT plan of care, and participate in the multidisciplinary care plan meetings including community staff, as appropriate.
- Educate and instruct clients, family members, or community staff, in therapy services and activities necessary to promote the client's health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a touch pad tablet.
Qualifications for a Physical Therapist include:
- A currentMassachusetts Physical Therapistlicense.
