The caseload is split roughly 50/50 between Salud Medical Center in Woodburn, Oregon, and Lancaster Family Health in Salem, Oregon. Mileage between work sites is reimbursed. Assigned clients will be within the Community Health Services Department with Maternity Case Management program and could include issues with grief, anxiety, depression, trauma, and people with emotional, behavioral, and social needs. In this role, you'll provide mental health services as determined in the treatment or care plan of the client. Support patients and families as they navigate medical challenges, process complex information about pregnancies and neonates, and access community supports. When perinatal loss occurs, helps families to understand, express, and cope with feelings of grief This position includes home visiting as part of the essential functions.
