The provider will round on inpatient acute heart failure (AHF) patients and provide ambulatory care for heart failure patients, seeing 15 patients per day (8-12 inpatient, 2-4 outpatient). The inpatient team includes an intermittent APP, while the office is supported by a customer service representative, MA, and RN.
