4 weeks of PTO 5-weeks paid time-of this consists of (4) 40 hour PTO weeks and (1)
40 hour CME week and $5,000 allowance and professional memberships per calendar year (which shall be prorated for any partial year worked by Provider) for costs and expenses related to provider's CME, including lodging, meals and travel incurred to attend CME courses.
Paid Sick Leave accrual (.0154/hr up to 40 hours/week)
Professional memberships examples would be for Provider's Washington State medical license fees, DEA permit fees, etc.
