Las Palmas Medical Center in El Paso, Texas is seeking a compassionate and experienced Registered Nurse Transplant Coordinator to join our Healthcare & Medical Services team. As a key member of our transplant program, you will play a central role in guiding patients and their families through every stage of the transplant journey while reflecting our commitment to high-quality, community-focused care.
In this role, you will coordinate the clinical and logistical aspects of transplant care from initial referral through long-term follow-up. You will conduct comprehensive nursing assessments, help determine transplant readiness, and ensure that required diagnostic tests, labs, and consults are completed and reviewed in a timely manner. Working closely with transplant surgeons, physicians, social workers, pharmacists, and other interdisciplinary team members, you will contribute to the development and implementation of individualized care plans tailored to each patient’s medical and psychosocial needs.
You will serve as the primary point of contact for transplant patients and their families, providing clear education on the transplant process, organ allocation, potential risks and benefits, and post-transplant expectations. This includes detailed teaching on medications, lifestyle changes, infection prevention, signs and symptoms of complications or rejection, and the importance of adherence to follow-up appointments. You will advocate for patients and help them navigate complex decisions with empathy and cultural sensitivity.
The Registered Nurse Transplant Coordinator will also collaborate with organ procurement organizations and internal stakeholders to coordinate organ offers, recipient matching, and surgical scheduling. You will help confirm patient readiness for transplant, facilitate communication between the transplant team and inpatient units, and support patients who present through the emergency department with transplant-related issues, ensuring smooth transitions and continuity of care.
This position requires meticulous attention to regulatory compliance. You will maintain thorough and timely documentation in the electronic medical record and transplant databases and help ensure adherence to institutional policies and external standards, including CMS, UNOS, and other governing bodies. You will participate in data collection, audits, and quality improvement initiatives aimed at improving patient outcomes, program efficiency, and patient and family satisfaction.
As part of Las Palmas Medical Center’s collaborative culture, you will work closely with the ER team and other departments, contributing your transplant expertise when patients require urgent or emergent interventions. You may participate in an on-call rotation to support time-sensitive organ offers and emergent patient needs. You will also engage in staff education, mentoring, and precepting for nurses new to transplant care, and may take part in community outreach related to transplant services and organ donation awareness.

This is a full-time, on-site position in El Paso, Texas.
