Minimum Qualifications
Associate degree in Physical Therapy from an accredited educational program.
Active Wisconsin PTA license required to start the assignment.
BLS certification required from the American Heart Association or American Red Cross.
Experience working in a skilled nursing facility or long-term care setting preferred.
Experience providing restorative or skilled rehab services to geriatric patients preferred.
Ability to follow treatment plans established by a supervising Physical Therapist.
Current immunizations and ability to pass facility background and health screenings as required.

Shift Name: Days.
Shift Details: Days 0830-1630.
Schedule Pattern: 5x8.
Floating: Candidate may be required to float to other facilities within the client network.
Contract Length: 13 weeks.
