Community Care Physicians, P.C., a leading multi-specialty medical group dedicated to delivering comprehensive, high-quality healthcare, is seeking a Nurse Practitioner or Physician Assistant to join our Healthcare & Medical Services department. In this role, you will provide patient-centered care across a diverse population, working in a collaborative, team-based environment that emphasizes quality, innovation, and compassionate service. You will perform comprehensive histories and physical examinations, assess and diagnose acute and chronic conditions, and develop evidence-based treatment plans in partnership with supervising or collaborating physicians. Responsibilities include ordering and interpreting diagnostic tests, prescribing medications and therapies within your scope of practice, and delivering preventive care, health maintenance, and patient education to support optimal health outcomes and patient engagement. You will utilize electronic health records for accurate, timely documentation and participate in practice-wide efforts to streamline care, enhance patient access, and ensure regulatory and organizational compliance. As part of Community Care Physicians’ culture of collaboration and continuous improvement, you will work closely with physicians, nurses, care coordinators, and other specialists to coordinate care, manage transitions, and contribute to population health initiatives and quality improvement projects. You will be encouraged to share clinical insights, adopt innovative approaches to care, and engage in ongoing professional development to advance your skills and stay current with guidelines and best practices. This position offers a hybrid work environment based primarily in our clinical offices with the potential for limited remote responsibilities aligned with organizational policies and patient care needs.
