Community Care Physicians, P.C., a large multi-specialty medical group dedicated to high-quality, patient-centered care, is seeking a Nurse Practitioner or Physician Assistant to join our Healthcare & Medical Services department within the Healthcare & Medical industry. In this advanced practice role, you will provide comprehensive clinical care to a diverse panel of patients, working collaboratively with physicians and an interdisciplinary team to deliver evidence-based, compassionate care across multiple specialties. Responsibilities include conducting detailed histories and physical examinations, ordering and interpreting diagnostic tests, formulating differential diagnoses, and developing and implementing individualized treatment plans. You will manage both acute and chronic conditions, prescribe medications and therapies in accordance with state and federal regulations, and coordinate care across settings to support continuity and optimal outcomes.
The Nurse Practitioner or Physician Assistant will play a key role in preventive medicine and population health by performing routine wellness visits, health screenings, and chronic disease management, as well as providing patient and family education focused on lifestyle modification, medication adherence, and self-management strategies. You will document all encounters thoroughly and accurately in the electronic health record, ensuring clear communication with the broader care team. Working within Community Care Physicians’ patient-centered culture, you will actively participate in quality improvement initiatives, adhere to clinical protocols and best practices, and contribute to projects that enhance efficiency, patient safety, and clinical outcomes.
