Mission Health, based in Asheville, NC, is seeking a Clinical Nurse Coordinator Pulmonary to join our Healthcare & Medical Services team. In this role, you will serve as a clinical leader and care coordinator for patients with pulmonary conditions, including COPD, asthma, interstitial lung disease, and other complex respiratory disorders. Working in collaboration with pulmonologists, respiratory therapists, nursing staff, and an interdisciplinary care team, you will help ensure safe, high-quality, patient-centered care from admission through discharge and follow-up.
The Clinical Nurse Coordinator Pulmonary will oversee and coordinate the day-to-day clinical operations of the pulmonary care area, facilitating patient flow, triage, and transitions of care while promoting evidence-based nursing practice. You will conduct comprehensive assessments, develop and implement individualized care plans, provide direct nursing care as needed, and serve as a clinical resource and mentor to bedside nurses. You will participate in patient education on disease management, medications, oxygen therapy, pulmonary rehabilitation, and lifestyle modifications, helping patients and families understand and engage in their care.
This position plays a key role in quality improvement and patient safety initiatives, including monitoring clinical outcomes, supporting adherence to clinical pathways and protocols, and assisting with performance improvement projects related to pulmonary care. The Clinical Nurse Coordinator Pulmonary will collaborate closely with providers to optimize care coordination, reduce readmissions, and enhance the overall patient experience. You will communicate regularly with patients, families, and the care team to coordinate appointments, diagnostic testing, respiratory therapy, and follow-up care, including transitions between inpatient, outpatient, and home settings.
