Requirements:

Active RN license in the practicing state.
Experience in home health, hospice, or palliative care preferred.
Strong clinical, communication, and patient-education skills.
BLS required; hospice or palliative care certification preferred.

Responsibilities:

Assess patient health status and develop individualized care plans.
Administer medications, treatments, and interventions safely.
Provide education and emotional support to patients and families.
Monitor patient progress and communicate changes to the care team.
Document care and ensure compliance with regulatory and quality standards.
