Requirements:

Active RN license in the practicing state.
BLS certification required.
Prior home health, case management, or community nursing experience preferred.
Strong assessment, clinical judgment, and care coordination skills.
Ability to work independently and manage a home-visit caseload.
Reliable transportation for patient visits.

Responsibilities:

Perform in-home patient assessments and develop individualized care plans.
Provide skilled nursing care including wound care, medication management, and health education.
Monitor patient progress and adjust care plans as needed.
Coordinate services with physicians, therapists, social workers, and caregivers.
Document assessments, interventions, and patient outcomes accurately and timely.
Educate patients and families on disease management, safety, and home care instructions.
Ensure compliance with regulatory, agency, and quality standards.
