Travel RN Job Details
- Position Type: Travel Registered Nurse – Home Health
- Location: Portage, Michigan 49002 (serving North/East Kalamazoo County; occasional coverage in Western Calhoun/Barry County)
- Schedule: Monday-Friday, 8:00am – 5:00pm
- Shift Duration: 8-hour day shifts
- Assignment Length: 13 weeks (Start: 10/13/2025, End: 1/11/2026)
- Hours per Week: 40
- Estimated Weekly Salary: $1920–$1970
Registered Nurse Qualifications
- Active Registered Nurse (RN) license in Michigan
- Minimum 1 year of recent Home Health RN experience
- Proficiency with HCHB (Homecare Homebase) and OASIS documentation systems
- Strong clinical assessment and critical thinking skills
- Excellent communication and organizational abilities
Home Health RN Responsibilities
- Deliver skilled nursing care to patients in their homes throughout Portage and Kalamazoo County
- Conduct comprehensive patient assessments and create individualized care plans
- Document patient care using HCHB and OASIS systems
- Collaborate with interdisciplinary healthcare team members for optimal patient outcomes
- Educate patients and families on disease management, medication administration, and preventive care
- Travel within North/East Kalamazoo County and occasionally support Western Calhoun/Barry County
