Job Description

The Opportunity:

Saint Luke's is seeking a compassionate Palliative Care Nurse to serve at Saint Luke's South Hospital. This part-time position will give you the opportunity to help patients and families at a very vulnerable time, supporting them through their goals of care and symptom management needs. You will be a part of a palliative care team that will work together to provide support to medical professionals in managing hospital inpatients or those benefiting from comfort care measures.

The Work:

Your skills as a Palliative Care Nurse will be challenged in a meaningful way as you will be responsible for:

Receiving pending referrals and palliative consults
Collaborating with a variety of team members including hospital staff, providers, post-acute care team members and the inpatient Palliative Care team members
Triaging patient needs
Providing goals of care conversations and resources
Educating staff, physicians, patients and families on palliative and/or end of life care
Facilitating transitions to next level of care, working especially with St. Luke's Hospice and Hospice House staff
Collecting, reporting, and monitoring key performance indicators of Palliative Care services
Looked to as the subject matter expert in palliative and end of life care in the inpatient setting

Missouri and Kansas RN License required.

Job Requirements

Applicable Experience:
3-5 years

Basic Life Support - American Heart Association or Red Cross, Registered Nurse - Various

Bachelor's Degree
