LanceSoft is seeking a travel nurse RN Care Manager for a travel nursing job in Novato, California.

Job Description & Requirements

Specialty: Care Manager

40 hours per week

Requirements:

Experience: Minimum 2 years must be recent Home Health Case Manager experience (hospital CM experience not applicable).
Travel experience: Required.
Certifications: CA RN License, BLS (AHA), valid driving license.
Charting: Epic required; OASIS experience mandatory.
Skills:Care coordination, Discharge planning, Plan of care development, CMS regulations, OASIS documentation, MCS OASIS
Work setting: Home Health
