Care Manager RN Part- time Days at Providence Health and Services summary:
The Care Manager RN is responsible for coordinating and managing patient care within a hospital or healthcare system to ensure optimal health outcomes and appropriate resource utilization. This role requires collaboration with multidisciplinary teams including nurses, social workers, and physicians to facilitate communication and care across the continuum. Candidates must hold an active RN license and have experience in acute care management or care coordination.
Description
Care Management in Hospital/Health Care Systems is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The Care Management process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of Care Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient's right to self determination. Required Qualifications:
- Bachelor's Degree
- Upon hire: California Registered Nurse License
- California Fire and Life Safety Card - National Organization within 30 days of hire
- 2 years experience in healthcare related field (Acute, Ambulatory, or Post-Acute).
- 2 years experience in Acute Care Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
Preferred Qualifications:
- Master's Degree in a Health-related/Science field
